Duties of the Division
01. Issue of certified copies (only from 9am to 3pm)
• Issue of certified copies of birth, death and marriages registered in the Colombo District before October 2016.
• Issuing certified copies of Birth / Marriage / Death registered by the Registrars of the Maharagama Divisional Secretariat from October 2016 up to date.

2. Handling declaration form (only 9.00 am to 2.00 pm)
• Registration of delayed birth registration (section 24)
• Registration of past deaths (Section 36)
• Amendments to the Name (Section 27)
(i) Less than 7 years
(ii) between 7 to 21 years
(iii) more than 21 years


  • • Amendment to information of father Section 27 (1) (4,7,9)
    • Amendments to mother's information regarding date of birth, sex in birth certificate and amendments to the death certificates in Section 52 (1)

General instructions for submission of declaration forms
• Birth, Marriage and Death certificates are the only certifies certified by Additional District Registrar of the Divisional Secretariat.
• It is compulsory to submit a stamped self addressed envelope of 9 × 4 size with every valid declaration form.

  • • Application forms should be submitted for obtaining copies after the revision (copies will be sent by post after the amendment).
    • To handover declaration forms one should come Before 2.00 pm
    • The receipt you get when submitting declaration form should be kept securely and the file number should be provided when inquiring about it.
    • affidavit should be certified by signing on a stamp to the value of Rs. 50 / - by a Justice of Peace or an Attorney-at-Law / Notary.

Registration fees

• Registration of the marriage 12 days after the submission of the marriage notice. - rs 950
• Registration of a marriage (on a special license) 12 days before submission of marriage notice. - Rs. 1150 / =.

Registration of Marriages outside the office
• Registration of the marriage 12 days after the submission of the marriage notice - Rs. 1000
• Registration of the marriage (on a special license) 12 days before handing the notice of marriage - Rs. 1200 / -

document needed when submitting a marriage notice
• Bride grooms birth certificate and a photo copy
• Brides Birth certificate and a photo copy
• A photo copy of the bride grooms national identity card
• A photo copy of the brides National Identity card
(Two persons with NICs are required to sign for evidence when submitting a marriage notice)

Amend the present name of the birth certificate.

For this, complete clause 27 of the B9 declaration form and submit it with the following documents.

• When the age is over seven and less than 21 years

1. A certified copy of the birth certificate.
2. A certified copy of the parents' marriage certificate.
3. A letter stating the information in the School Entry Register (from the office)
4. Three Documents with the name expected to be amended and which have been issued more than one year ago and their photo copies.

  1. 5. An affidavit of the applicant regarding the change of name.
    6. A recommendation letter from Grama Niladhari regarding the change of name.
    7. If the declarer is mother a letter from the father expressing the consent
    If the declarer is the father a letter from the mother expressing the consent

    (The declaration form should be submitted by the mother or father and if the child is parentless a guardian who has been appointed by a court of jurisdiction, an elderly brother or sister, grandmother grandfather, a brother or sister of the parents, must submit the declaration form).

    After paying Rs. 50 / - as the declaration form fee, receipt of A86 should be obtained

Amend the present name of the birth certificate.

For this, complete clause 27 of the B9 completed declaration form and submit it with the following documents.

• When the age is less than 07 years

1. A certified copy of the birth certificate.
2. A certified copy of the parents' marriage certificate.
3. Child's Health Record Book and a photocopy of it
4. Applicant's affidavit of the change of the name of the child.
5. Three Documents with the name expected to be amended and which have been issued more than one year ago and their photo copies.


(The declaration form should be submitted by the mother or father and if the child is parentless a guardian who has been appointed by a court of jurisdiction, an elderly brother or sister, grandmother grandfather, a brother or sister of the parents, must submit the declaration form).

After paying Rs. 50 / - receipt of A86 should be obtained

Amend the present name of the birth certificate.

For this, complete the clause 27 Declaration form B-10 and submit along with the following documents.

• When the age is over 21 years


  1. 1. A certified copy of the birth certificate.
    2. A newspaper ad
    3. A recommendation letter from Grama Niladhari regarding the change of the name
    4. Applicant's affidavit of the change of the name.
    5. Three Documents with the name expected to be amended and which have been issued more than one year ago and their photo copies.

(Declaration should be submitted by ownself.)

After paying Rs. 50 / - receipt of A86 should be obtained

Making amendments in Birth Certificates.

• 27 (a) If the declarer is the father his information in the line 4,6,7,8 and 9 in the birth certificate can be amended by a (27)අ declaration form. For this (27) ආ 37 අ declaration form should be completed and submitted with the following documents.

1. A certified copy of the birth certificate
2. A certified copy of the birth certificate of the father
3. A certified copy of the parents' marriage certificate
4. If there is any difference in the name of the father's birth / marriage certificate, an affidavit for it

• If the informant in the declaration form 52(1) is the mother, amendment can be made to line 1,5 and 9 in the birth certificate.In order to do so, 52(1) clause be 61 declaration form should be completed and submitted with the following documents.

1. A certified copy of the birth certificate
2. A certified copy of Mother's Birth Certificate
3. A certified copy of the parents marriage certificate
4. If there is any difference in the name of the mother's birth / marriage certificate, an affidavit for it

After paying Rs. 50 / - receipt of A86 should be obtained

Amendment of death certificates.

Complete the declaration form 52 (1) clause B61 and submit with following documents

1. A certified copy of the death certificate
2. Documents that can support expected amendment
Eg.
The Birth Certificate of the dead person

Marriage certificate of the dead person(These should be certified copies)
3. Recommendation letter from Grama Niladhari
4. An affidavit
5. A Photocopy of the declarer’s Identity Card


After paying Rs. 50 / - receipt of A86 should be obtained

Registration of an unregistered birth

For this purpose, complete and submit 24 clause B6 declaration form and the affidavit with the following documents  


1. Hospital Birth Information Sheet
2. A child's health record book and a photocopy of the parts which include the information of the child.

  1. 3. Certified copy of father’s birth certificate
    4. Certified copy of Mother's Birth Certificate
    5. Certified copy of the parents' marriage certificate
    6. List of children in the family and photocopies of birth certificates of them
  2. 7. If a year has elapsed after the birth and empty result sheet which showing that the birth is not registered

    (The affidavit should be signed on a stamp to the value of Rs. 50. A86 should be obtained by paying rs 50 as declaration form fee. When empty result sheet is needed Rs 200 should be paid with B63 application form and obtained and A86 receipt.
